T&E Test Accommodation Information for Students
Testing and Evaluation Services offers test accommodation services for students with disabilities who require special accommodations that are not the standard time and space accommodations. T&E does not provide accommodations that merely require extended time or private rooms. These types of accommodations should be handled by the instructor. The Testing and Evaluation Services Alternative Testing Center is located in Room 341 Educational Sciences Building, 1025 West Johnson Street (at the corner of West Johnson and Brooks Streets). The staff would like to make your testing experience as pleasant as possible. To ensure this, we ask that you follow the procedures listed below.

This service is for classroom testing only. Comprehensive exams, prelims and other high stakes assessments cannot be scheduled through this service and need to be taken through the department.

If using T&E for proctoring your exams, please remember there will not be anyone available to answer questions regarding the content of the exam.

Obtain all exam dates for the semester when you meet with your instructors during the first three weeks of the semester. Determine which exams the faculty will proctor and which ones T&E will proctor.

To request accommodated testing at Testing and Evaluation Services, please use the link below to access a request form. Print off one form for each exam request, fill it out, get the instructor’s signature, and bring the forms to Testing and Evaluation Services, room 373 Educational Sciences Bldg at least seven days before a scheduled test. The forms can be faxed to our office at (608) 263-4291. If you drop the request forms off after business hours (8:00am – 4:30pm) you may use the locked mailbox #308 directly across from the elevator.
We strongly recommend doing this within the first three weeks of the semester. Early notification will help you, your instructor and T&E staff in planning for the semester. Exam requests must be submitted no later than three days in advance of each exam.

If you need an audio exam, let the instructor know that the instructor needs to provide a tape-recorded copy of the exam or an elecronic copy to be used with speech output software (e.g., Kurzweil or JAWS). If you'll be using an electronic copy, make sure you request Kurzweil or JAWS when filling out the form. Arrange training on the software well in advance of your exam.

If you are using voice recognition software (Dragon Naturally Speaking), arrange training on the software well in advance of your exam, if needed, and ensure that your voice files have been copied to T&E's computer.

Contact your instructor and then the T&E office as soon as possible if you need to change or cancel an exam. T&E needs written confirmation from the instructor to change the exam time or date.

Please be on time! If you arrive late, you may not be able to test at that time.

We do not have facilities for studying, so you will not benefit by arriving early for that purpose.

All materials, books, backpacks, etc., should be placed under your desk. Only the items specified on your alternative testing request form will be allowed on your desk during testing.

Used scratch paper and test booklets must be given to the proctor at the end of the testing session. You may not keep these for your notes unless specified in writing by your professor.

Please clean up your testing area before you leave.
